Every individual deserves to live in a home of his or her choice

ENTRANCE CRITERIA

Supported Living Services (SLS) is provided to an individual that meets the following criteria.

​

  • Male or female, ages 18-59 yrs with an ambulatory or non-ambulatory status. 60+ years of age must have Regional Center prior approval.

  • Consumer must have a diagnosis of  Mild to Moderate Intellectual Disability, Cerebral Palsy, Epilepsy, Autism, or any other developmental disorder recognized by the Regional Center.

  • Individual must require supervisor and assistance for behavioral excesses as well as for moderate deficits in self-help skills and adaptive functioning.

  • ICS - SLS will consider accepting individuals who have medical conditions that do NOT require a 24 hour nursing care, transition from a State Developmental Centers or a locked facility to a Supported Living Environment. ICS - SLS shall review the individuals circumstances with the Regional Center on a case-by-case basis.

SUPPORTED LIVING SERVICES

MAKING CHOICE OUR PRIORITY

Inclusion Specialized Supported Living Services (IS SLS) will train and support the individual with all the necessary daily living skills in the individuals's natural community and home environment. Some skills include, cooking, cleaning/organizing, budgeting and money management and sustaining a healthy living environment.​
